End-of-life is a mix of bittersweet, difficult, beautiful and precious moments. At Triniti Home Health & Hospice, we are committed to giving our hospice clients the most comfortable and honoring end-of-life journey possible.

We understand the joys and challenges of the aging process, and offer you 3 decades of experience coming alongside clients as they live out their golden years. Triniti Home Health & Hospice is jointly owned by Guardian Angels Senior Services and Cassia, an Augustana/Elim affiliation. We serve people in Anoka, Carver, Dakota, Hennepin, Isanti, Mille Lacs, Ramsey, Scott, Sherburne, Washington and Wright counties in Minnesota. We are consistently named in the top 25 percent of home care agencies nationwide for our excellent outcomes.

At Triniti Home Health & Hospice we are committed to giving our hospice patients the most comfortable and honoring end-of-life journey possible. Hospice care is for individuals who are in their final stages of life or who wish to focus on comfort rather than a cure.

We recognize the unique and difficult challenges of terminal illness, and our goal is to provide emotional, physical, and spiritual support to individuals and their families. Hospice services are provided in the client’s home, whether it be a private home with family, a care center, or assisted living facility.

Each client is assigned a nurse case manager who oversees the plan of care from start to finish, increasing visits as needed depending on the client’s condition. In addition, our medical director is certified in hospice care and remains available to the hospice team 24/7. aid in the treatment we give our clients.

Our hospice agency standards continue to impress our clients with our experienced staff and extended weekly visits. At Triniti Home Health & Hospice, we visit our clients twice as often as the regulated guidelines, as well as implement our own music therapist and massage therapist to aid in the treatment we give our clients.

One of the many ways in which Triniti helps patients with chronic conditions or post-surgery recovery is with physical therapy. Through physical therapy, patients feel better with less pain, increased relaxation, and reduced need for medications.
